These Terms and Conditions govern your use of the Comfort Sole Foot Care website and your engagement with the services provided by Comfort Sole Foot Care, a trade name (DBA) of A Nurse's Journey LLC. By using this website or requesting our services, you agree to these terms.
Services Provided
Comfort Sole Foot Care provides mobile foot care nursing services performed by a licensed Registered Nurse. Our services are within the scope of nursing practice and are not a substitute for medical care, diagnosis, or treatment by a licensed physician or podiatrist. We do not diagnose medical conditions or prescribe treatments.
Appointment Scheduling and Cancellation
Appointments are scheduled by mutual agreement and confirmed in advance. We ask that you provide at least 24 hours' notice if you need to cancel or reschedule. We reserve the right to charge a cancellation fee for late cancellations or no-shows, which will be communicated at the time of scheduling.
Payment
Comfort Sole Foot Care is a private-pay mobile RN foot care practice. All services are private-pay and payment is due at the time of service. We do not bill insurance or assist with insurance reimbursement. Accepted payment methods are communicated at the time of scheduling.
Patient Responsibilities
You agree to provide accurate and complete health history information prior to and during your visits. You understand that withholding relevant health information may affect the safety and quality of care provided. You are responsible for following any at-home care recommendations provided by your nurse.
Limitation of Liability
To the fullest extent permitted by law, A Nurse's Journey LLC and its practitioners shall not be liable for any indirect, incidental, or consequential damages arising from the use of our services or website. Our liability for any claim arising from services rendered shall not exceed the amount paid for the specific service giving rise to the claim.
Scope of Practice
Our services are provided within the scope of Registered Nursing practice as defined by the Ohio Board of Nursing. We do not perform services that fall outside this scope. If a concern is identified that requires evaluation or treatment beyond nursing scope, we will advise you to seek appropriate medical care.
