Who We Are
Comfort Sole Foot Care is a trade name (DBA) of A Nurse's Journey LLC, an Ohio limited liability company. We are a covered entity under the Health Insurance Portability and Accountability Act of 1996 (HIPAA) and its implementing regulations. This Notice of Privacy Practices describes how we may use and disclose your protected health information (PHI) and your rights regarding that information.
What Is Protected Health Information?
Protected health information (PHI) is individually identifiable health information that we create, receive, maintain, or transmit in connection with providing nursing services to you. This includes information about your health history, current health status, medications, and the care we provide.
How We May Use and Disclose Your PHI
We may use and disclose your PHI for treatment, payment, and healthcare operations without your written authorization. We may also disclose PHI as required by law, for public health activities, health oversight activities, and to prevent serious threats to health or safety. All other uses and disclosures require your written authorization, which you may revoke at any time in writing.
Your Rights Regarding Your PHI
You have the right to access and obtain a copy of your PHI, request amendments to inaccurate information, request an accounting of certain disclosures, request restrictions on use or disclosure, request confidential communications, and receive a paper copy of this Notice. To exercise any of these rights, please contact us in writing.
Our Duties
We are required by law to maintain the privacy of your PHI, provide you with this Notice, abide by its terms, and notify you in the event of a breach of your unsecured PHI. We reserve the right to change this Notice and will post any revised version on our website.
Complaints
If you believe your privacy rights have been violated, you may file a complaint with us or with the U.S. Department of Health and Human Services Office for Civil Rights at www.hhs.gov/ocr/privacy/hipaa/complaints. We will not retaliate against you for filing a complaint.
